The
Progressive Care
Center at Deaconess is designed to help people
transition back to everyday life following an illness or injury which
required an acute hospital stay. Often referred to as transitional care, the
19-bed Medicare Skilled Nursing Facility offers medical, nursing, and
rehabilitative treatment to its patients.
Skilled nursing care is provided for patients who no longer need acute
hospital services, but who meet Medicare criteria for require specialized
nursing care and/or therapy services. Physical, occupational and speech therapy
services are provided on the nursing unit, as well as social services, patient
education and activity programs. Patients have access to a communal dining area
and a large day room, which includes a television, VCR and DVD. Call (405)

- 24 hour nursing care

- Diabetic monitoring and education
- Pain management
- Leisure activities
- Dietary consultation and management
- Trachea care
- Social services
- Family Conferences
- Patient and family education and training
- Discharge Planning
- Referral for aftercare services upon discharge
